Access and Referral Information

How do I get referred into the ACCORD Inpatient Unit?

Hospice care is provided free of charge for all and is available to patients aged 18 years and over who have been diagnosed with a progressive life limiting illness.

We accept referrals for:

  • Management of complex physical symptoms
  • Management of complex emotional / spiritual distress
  • Multidisciplinary Team (MDT) assessment and rehabilitation
  • End of life care

The Hospice operates an equality and diversity policy such that no patient shall be disadvantaged in any aspect of their care as a consequence of cultural, racial, sexual or social characteristics or any form of handicap.

Any healthcare professional can refer patients to the hospice, providing the referral has been discussed with and agreed to by the patient.

Please also ensure that the patient’s GP or hospital Consultant is aware of the referral to the hospice. If you are a healthcare professional wishing to make a referral please visit our Referral System below
